Welcome to this module on Chronic Condition Self-Management (CCSM).The aim of this learning package is to support GP trainees in facilitating self-management of patients with a chronic condition. It contains a number of lessons that provide background information and outline a framework for effective interventions. This is a secure website for Irish GP trainees and requires registration.

Video lessons are approximately 10 minutes long to facilitate your learning. Supplementary lessons, where non-core information is presented, and relevant printable and downloadable material and notes are also included.

        The six lessons are as follows:
        • Lesson 1: What is Chronic Condition Self-Management?
          This lesson provides an overview of the concept of CCSM and of how self-management can impact on care and patient outcomes.
        • Lesson 2: Adopting a Self-Management Approach
          This lesson summarises the rationale for adopting a self-management approach, and highlights some of the indications and challenges for CCSM. A number of self-care principles are also described.
        • Lesson 3: Self-Management Interventions
          This lesson presents some common models of self-management. Core skills that are required to undertake or to support self-care are also outlined.
        • Lesson 4: Skills for Eliciting Behaviour Change
          This lesson provides a description of skills that can be used to encourage behaviour change, focusing in particular on Motivational Interviewing.
        • Lesson 5: A Framework for Self-Management Support
          This lesson focuses on improving quality of life in chronic illness through patient empowerment, shared decision-making and managing access to supports.
        • Lesson 6: Adherence to Medication
          This lesson highlights the challenges of poor adherence to medication.

The Medical Council has defined eight domains of good professional practice. These domains describe a framework of competencies applicable to all doctors across the continuum of professional development from formal medical education and training through to maintenance of professional competence. Since they describe the outcomes which doctors should strive to achieve, doctors should refer to these domains throughout the process of maintaining competence. In order to assist you in planning your professional competence, ICGP eLearning highlight the domains addressed in each module as set out below. 
NOTE: While a number of ICGP modules have a tight focus on one or two domains, most modules endeavour to address all domains to some degree (low, medium, high). This is clearly displayed in the diagram below
